I'm sure a lot of stuff concerning things like patient records is just red tape and lack of communication between departments - happens in every business. When I was having surgery regularly, X ray pics were still physical things and they would hand me a file so heavy I could barely lift it. Now the consultant just mouse-clicks and there they are on a screen, so I don't see why it isn't the same with all records.
