I have that problem all the time. I have big boobs but narrow shoulders, so often if things fit me in the chest they fall off my shoulders. There is a company called Pepperberry that make clothing sized according to how, uh, voluptuous you are, but they are expensive and the range isn't that big. Still, it gives me hope that more companies might start doing the same.
