In the past he's accidentaly let the pre-record go out on air while it was being recorded (if you know what I mean?). A record was on and he was pre-recording the next link (it was a phone call) and it could be heard at the same level as the record.

i remember this he just didnt turn his mic off while answering the phone...but he wasnt prerecording as he talked to the caller after the record

He does pre-record calls. I remember once when Simon Mayo ran down to the studios after he'd been on the toilet (apparently) to tell Moyles his mic was going out on air while a song was playing. After the record had finished, Moyles said something along the lines of "No point playing that call back, you've already bl00dy heard it".
