Tabloid papers have been rife with reports in recent days that Ball's marriage had broken down because of her relationship with another DJ, Dan Peppe the drummer with dance group Themroc.
Ball, who separated from Cook at the end of last year, said she was going public in a bid to keep her private life out of the press.
"I left Norman at the beginning of December. There were problems in our marriage that had arisen and I had subsequently met someone else," Ball, a former Radio One DJ, said in a statement released on Tuesday.
"I told Norman in December that this was the case. He didn't want to know the name or any other detail. My friendship with Daniel Peppe has developed since then," she said.
Cook, who first found fame as a member of 1980s band "The Housemartins" before switching to mixing discs under the alter ego of Fatboy Slim, married Ball in 1999.
He told the News of the World that he was willing to forgive Ball and wanted her back.
"I'm glad he (Peppe) wasn't my best friend or anything like that," he was quoted as saying.
Ball, who is taking a two week break from her Xfm radio show, appealed to the media for privacy asking for herself, Cook, Peppe and their families to be left in peace.
